AFTER REGISTERING AND RECEIVING AN INVOICE, then COMPLETE EVERY SECTION of this form for CREDIT CARD PAYMENTS ONLY.

PLEASE NOTE:

ONE FORM PER EVENT.
Do NOT ever combine payments from different events into this form.
Make sure you are entering accurate information for the AMOUNT DUE from either Formstack or SpeechEase registration invoice.
It is the Troupe Director's responsibility to enter the CORRECT and ACCURATE AMOUNT into this form.
The 3.4% credit card fee will be added automatically and is NOT reflected in the initial registration invoice.
Do NOT add the credit card fee.
Please note: NO REFUNDS VIA CREDIT CARDS. We don't have that capability.

By checking this box, I understand that I am using this form ONLY FOR A CREDIT CARD PAYMENT for an Oregon Thespians event and it does NOT register me or my troupe or competitors AT ALL. Depending on the event, I must register through Formstack or SpeechEase FIRST and then use this form for a credit card payment. I also understand that the amount I must enter into this form is the TOTAL DUE listed on the invoice. The 3.4% credit card fee automatically will be added within this form. I should not add anything other than the registration invoice total at all. If I enter the invoice total + credit card charge BEFORE payment in this form, I will be double charged the credit card fee and CANNOT be refunded for my error.

If I make an error, I will need to correct my error when possible. (i.e. If I underpay, I will pay the remaining balance. If I overpay, I will not receive any refund.)
